We have many requests, from customers who have already booked us, for another copy of their “Information Sheet”.  That was the colored sheet with music info on one side and the bridal party on the back.  If you are having a party instead of a wedding, just use the first page.

There are several ways you can receive an extra copy on-line:

1) Just click here and print the page that loads. Probably the easiest even though it might not look the best.  You can also copy the page into your favorite word processing program and adjust and print it.

2) Click here and download a copy of the file in Microsoft Word format.

3) Click here and download a copy as a PDF file. You must have the free Adobe Acrobat Reader to do this.

If none of these work, just give us a call at 630-961-8306 and we will mail you another copy.
